Volunteer Screening Application
The first step in the application process is filling out a Kansas 4-H Volunteer Application Form. This form will provide the Extension Office with basic information about you. The information you must give on the form includes:
Includes information such as your name, address, driver's license number, phone number, and previous 4-H or youth group involvement. It also includes your volunteer interest, such as why you want to be a volunteer, time commitment desired, type of volunteer position desired, and previous work and volunteer experience.
This includes information such as your driver's license, date of birth and vehicle insurance coverage. It also requests any history of substance abuse, criminal behavior, and child abuse and neglect. You will need to provide three references and the Extension Office will follow up with the references given. You also have the option to turn in completed reference forms which are signed by the referrer with your application. This will speed up the process if you are needing to become a registered volunteer in a timely manor. Click Reference Form to view it in a form fillable PDF.
It is very important that you fill the Application out completely. Not doing so will result in a delayed process.
The application can be completed online or a form fillable application can be completed and printed out.
